Where you'll be working
The Emergency Department to Community Team is part of Primary and Community Health. The office is located at Macquarie Hospital North Ryde. The CNC will be working across all NSLHD Emergency Departments and in clients' homes.
What you'll be doing
The Emergency Department to Community (EDC) program works closely with adults frequently presenting to emergency departments across Northern Sydney Local Health District (NSLHD). EDC provides tailored intensive case management and specialist care to clients in the community, improving their health and reducing the need for emergency department presentation.
People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.
Candidates will need to meet the following criteria:
- Registered with Authority to Practice with Australian Health Practitioner Regulation Agency. Minimum seven years full time post graduate experience with 5 years full time equivalent in the specialty field. Specialty field relevant to the EDC program includes mental health, drug and alcohol, pain service and/or community health. In addition, the applicant must have an approved post graduate qualification (minimum Grad Diploma/Certificate level) or evidence of working towards Masters’ level qualification.
- Ability to engage complex client groups in care coordination to plan and undertake activities to deliver client-centered goal plans.
- Current unrestricted Class C NSW Driver’s Licence.
