What you'll be doing

The Child & Family Health nurses (CFHN) offer the Sustaining NSW Families (SNF) nurse-led home visiting program for families with moderate levels of vulnerability from mid-pregnancy until their child is two years of age. The SNF Program aims to support healthy parent-child relationship development and support new parents' capacity to optimise their infant's health, well-being, physical, emotional, cognitive, communication, and social development.

The CYF SNF Program is a nurse-led multidisciplinary program offered as part of the Child and Family Health Service within the broader Northern Sydney Local Health District (NSLHD) Child, Youth and Family (CYF) Community Health Service.

The CFH SNF nurses engage with families using a strengths-based approach, supported by a multidisciplinary allied health team. The SNF nurses offer a series of evidence-based, structured interventions that aim to foster positive parent-infant relationships, nurture parent-child attachment and optimise children’s development. The CFHN will complete the advanced practice (SNF) training course (if not prior) within 18 months of starting to support their practice in the Program.

People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.

Candidates will need to meet the following criteria:

  1. Registered Nurse with Australian Health Practitioner Regulation Agency (AHPRA) and with a recognised post-graduate Child & Family Health Nursing (CFHN) qualification.
  2. Experience working with vulnerable families and a demonstrated understanding of early intervention, including the theoretical basis that underpins sustained home visiting, prevention, and community development.
  3. Demonstrated experience working with vulnerable families in the family partnership model, including those from diverse cultural and linguistic back grounds.
  4. Demonstrated effective clinical communication through the use of information technology and sound written verbal and interpersonal skills.
