Delivering acute care in the patient's own home - Hospital in the Home (HITH) treats acute patients in the home and in the clinic, enabling recovery within their own home and surrounded by family and carers.

Where you'll be working
We are a district wide service and supporting our patients within the Northern Sydney Local Health District. Our team are located primarily at Royal North Shore, Mona Vale and Hornsby. This role will be located at Royal North Shore Hospital campus.
What you'll be doing
People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.
Candidates will need to meet the following criteria:
- A Bachelor or post graduate degree in Social Work with eligibility for the Australian Association of Social Workers (AASW). Level 2 Social Workers must show evidence of minimum three years’ experience in a health care setting working as a qualified Social Worker.
- Demonstrated understanding of Hospital in the Home model of care. Demonstrate skills and knowledge of a Social Worker in the provision of service to consumers, carers, and families requiring assessment, counselling, crisis management, advocacy, discharge planning, and appropriate referrals.
- Proven ability to work independently with minimal supervision and as a collaborative member of the multidisciplinary team, in a health care environment, with competing demands. Explain the key considerations for a social worker who works in a multidisciplinary team delivering acute care to patients in their own home.
- NSW unrestricted drivers’ licence.
