This position will work with the Community Health Diabetes Team. The role involves community and inreach into the acute sector across Tweed Byron Community & Allied Health.


This vacancy relates to maternity leave relief for an initial period of up to 36 weeks for 24 hours per week; the hours and period of employment may change according to the maternity leave requirements.  There is also the possibility of an extension at same / reduced hours.

What you'll be doing
The Registered Nurse coordinates and delivers high quality person-centred nursing care in accordance with the Nursing & Midwifery Board of Australia Standards for Practice for Registered Nurses, and the Policies and Procedures of the MoH and NNSWLHD.
  
The Registered Nurse Diabetes Educator is responsible for the provision of safe, efficient and effective health interventions related to diabetes education and management within the community and hospital in- patient settings and to be a clinical resource person to the community and other health professionals in the area of diabetes.
  • Current registration with Nursing & Midwifery Board of Australia as a Registered Nurse (Division 1); ADEA recognised course in the management of diabetes; working towards credentialing as a Diabetes Educator.
  • Demonstrated effective communication through the use of information technology, written, verbal and interpersonal skills with the ability to identify key messages, issues and concerns when communicating with others.
  • Knowledge and understanding of person-centred care, evidence-based practice and quality improvement.
  • Demonstrated understanding of and ability to practice in accordance with legislation, policies and within the scope of practice of a Registered Nurse.
  • Knowledge and recent clinical experience in all aspects of management for clients with diabetes in both community and in-patient settings.
  • Understanding of the principles of primary health care practice and integrated care and their applicability to service delivery.
  • Demonstrated ability to use problem solving skills and a multidisciplinary team approach in the planning, delivery and coordination of patient care.
  • Valid unrestricted drivers licence for use in NSW/Australia.
