Nurse Practioner/Transitional Aged Care - Port Macquarie Base Hospital

1. Current registration with the Nursing and Midwifery Board of Australia with endorsement to practice as a Nurse Practitioner or currently working toward endorsement as a Nurse Practitioner and ability to complete requirements within a 2-3 year period.
2. At least 5 years full time equivalent post registration nursing experience with extensive clinical experience in aged care and ability to function at an advanced autonomous level.
3. A post graduate qualification relevant to the clinical speciality - Aged Care/Dementia/Chronic Care
4. Demonstrated comprehensive skills in clinical assessment including physical, mental health, social, ethnic and cultural dimensions.
5. Excellent leadership and interpersonal skills with proven ability to use appropriate teaching/learning strategies to educate and support others within the speciality and the profession.
6. Proven ability to critically evaluate and integrate relevant research findings into decision making about health care management and interventions.
7. Evidence of implementing interventions and monitoring outcomes to evaluate and improve patient care
8. Current NSW driver's licence and ability and willingness to travel for work purposes
