Acute to Aged Related Care Co-ordinator AARCS – Health Clinician Lvl 2- Coffs Harbour Health Campus – PFT

1. Current Registration with the Occupational Therapy Board of Australia; or Bachelor of Social Work and eligible for membership to the Australian Association of Social Workers; or Current Registration with the Psychology Board of Australia; or Current Registration with the Nurses and Midwives Board of Australia; or current registration with the Physiotherapist Board of Australia
2. Demonstrated ability to use problem solving skills and a multidisciplinary team approach in the planning, delivery and coordination of patient care.
3. Demonstrated effective communication through the use of information technology, written, verbal and interpersonal skills with the ability to identify key messages, issues and concerns when communicating with others.
4. Knowledge and understanding of person-centred care, evidenced based practice and quality improvement.
5. Demonstrated understanding of and ability to practice in accordance with legislation, policies and within the scope of practice of your discipline.
6. Understanding of the principles of primary health care practice and integrated care and their applicability to service delivery
7. Demonstrated understanding of Aged Care Assessment and/or Aged Care service delivery.
8. Valid unrestricted Drivers Licence for use in New South Wales/Australia.
