About the Opportunity
COPILOT is an exciting service that provides rapid access to multi-disciplinary geriatric assessment (medical, nursing, and allied health) for older people living in the community setting. Referral enables care to be implemented in the patient’s home environment, avoiding unnecessary transfer to hospital, and leading to improved patient outcomes.

The Senior Dietitian is responsible for providing patient centred and high quality nutrition services to clients in the community with the aim to keep patients well in the community, reduce unnecessary hospital admissions and facilitate early, safe discharges from hospital. The position will provide nutritional care to patients as per caseload; provide and participate in education and evaluation; supervise students and Level 1-2 dietitian as appropriate; and participate in ongoing professional development.



What You'll Be Doing

Where You'll Be Working 

SWSLHD Primary & Community Health provides a comprehensive range of community-based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney. 

These services include prevention, early intervention, assessment, acute and post-acute treatment, health maintenance, and continuing care. The team supports diverse populations, offering specialised programs for children, young people, older adults, and at-risk communities. 

With over 800 dedicated staff, including Aboriginal Health Workers, nurses, doctors, and allied health professionals, the service ensures high-quality care close to where people live.

  1. Post graduate qualification or Bachelor in Nutrition and Dietetics as recognised by the Dietitians Association of Australia.
  2. Relevant clinical experience including demonstrated advanced practice skills and management of complex case load to clients in a community setting and with chronic diseases.
  3. Excellent written and verbal communication skills, including group education skills and excellent computer skills including CBORD.
  4. Demonstrated effective time management skills and ability to work independently and as part of a multidisciplinary team.
  5. Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
