  1. Registered Nurse with the Nursing and Midwifery Board of Australia with post graduate qualifications relevant to Chronic Care and at least 3 years’ experience working in a clinical field related to a broad range of Chronic Disease Management.
  2. Understanding of the Agency for Clinical Innovation (ACI) Osteoarthritis Chronic Care Program (OACCP) Model of Care, and best practice standards for the assessment, and management of (hip and knee) joint pain.
  3. Demonstrated experience at an advanced level of clinical expertise in assessment, care planning and care provision of patients with chronic pain, chronic disease conditions and/ or complex social needs, to support good patient care outcomes.
  4. Demonstrated understanding of osteoarthritis, orthopaedics and joint replacement surgery in the pre-operative, acute care and post-operative phases.
  5. Evidence of clinical leadership skills and the ability to work collaboratively within a multidisciplinary framework to plan, deliver and coordinate patient care.
  6. Demonstrated commitment to excellence in nursing practice, the provision of education to others and own professional development.
  7. Demonstrated effective communication through the use of information technology, written, verbal and interpersonal skills with the ability to identify key messages, issues and concerns when communicating with others.
  8. Valid unrestricted drivers licence for use in NSW/Australia and willingness to travel in the course of employment.
