About Us

Hawkesbury District Health Service is a teaching hospital in Windsor NSW, providing acute, community and allied health services for the local community and surrounding districts. The service provides 24-hour emergency, medical, surgical, diagnostic, maternity, neonatal, paediatric, palliative, intensive and coronary care services.

 

The Hawkesbury Transitional Aged Care Program provides home based therapy for clients living within the Hawkesbury LGA. The Transitional Aged Care Program provides low intensity therapy and support services for older adults in their own homes following an inpatient stay in hospital. TACP service focuses on restoring, improving, or maintaining older people’s independent functioning and wellbeing.  The focus of this service is assisting older people to regain or maintain physical, functional and cognitive abilities which support them to either maintain or recover a level of independence, allowing them to remain living in the community. This position is permanent part time position working 16 hours per week as a member of a multidisciplinary team.

What you will bring to the role:

  1. Degree in Physiotherapy and current National Physiotherapy Registration with AHPRA with minimum 2 years clinical experience.
  2. Demonstrated advanced clinical knowledge, reasoning, skill, and community clinical multidisciplinary team experience in the provision of Physiotherapy in the treatment of older people.
  3. Demonstrated high-level verbal and written communication and interpersonal skills, including the ability to develop care plans and adapt communication style to suit different audiences.
  4. Demonstrated ability to exercise independent professional judgement in the provision of clinical care using evidence-based practice, commensurate with years of experience.
  5. Demonstrated ability to work effectively in a multidisciplinary team.
  6. Demonstrated participation or leadership in quality improvement processes and teaching, commensurate with years of experience
  7. Ability to respond to the demands and pressures of the role while managing time effectively.
  8. Current Driver’s Licence and willingness to travel in accordance with the demands of the position
