Vaccination Requirements
This is a NSW Health Category A position, which requires immunisation the full list of vaccinations outlined in the NSW Health OASV Policy.
If you have any current evidence of vaccination against these, upload them with your application. Your current vaccination status will not impact your application. If you are the preferred candidate, you will be responsible for covering the costs of any remaining vaccination requirements. The Staff Health Team will access your AIR record if needed, please advise if you prefer the team not to view your vaccination record.
 
You will be required to complete the OASV Undertaking/ Declaration Form and TB Assessment Tool during the recruitment process. Please upload this with your application.
 
COVID-19 Vaccination Compliance
All NSW Health workers are required to have completed an up-to-date course of a COVID-19 vaccine (3 doses for Category A roles, 2 doses for Category B roles) which has been approved or recognised by the Therapeutics Goods Administration (TGA). New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate certifying the worker cannot have any approved COVID-19 vaccines available in NSW.
 
Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Please provide proof of vaccination if available. The Staff Health Team will access your AIR record if needed, please advise if you prefer the team not to view your vaccination record.
