Clinical Nurse Specialist - Complex Care Coordinator

2. Registered Nurse (Division 1), with current AHPRA registration with a minimum three (3) years clinical experience in Complex Care Coordination, relevant post registration qualifications and advanced clinical skills in patient assessment and care planning.

3. Demonstrated ability to communicate effectively with patients, carers and a diverse range of health care providers including those in government, non-government and residential aged care and community organisations.

4. Demonstrated ability to work well within a multidisciplinary team, with an emphasis on service delivery, quality of care and team cohesion.

5. Experience or interest in participating in translational research, working together with patients, clinicians and researchers to undertake pilot / proof of concept studies aimed at refining existing practices, developing new care pathways and improving health outcomes.

6. Sound experience using Microsoft Office applications and evidence of personal time management capabilities and effective negotiation skills; with a current NSW driver’s licence.

7. Experience in practice that promotes self-management support, behaviour change and psychosocial support and a strong understanding of the health and psychosocial issues for people living with chronic conditions and their carers.

8. Ability to identify and improve opportunities to enhance continuity of care between acute hospitals, primary care providers and community based services and a comprehensive knowledge of primary health and community based services.
