  1. Registered Nurse with the Nursing and Midwifery Board of Australia and a minimum of 5 years’ full time equivalent post registration experience and at least 3 years’ full time equivalent experience in Chronic Disease Management (CDM) in diverse clinical settings and a post graduate qualification in Chronic Disease Management (CDM) or other area relevant to the specialty.
  2. Demonstrated experience working with local aboriginal communities and an understanding of the relationships that this requires working with the Aboriginal Medical Services and other Aboriginal support services particularly in relation to the care of the patient with chronic conditions.
  3. Demonstrated understanding of Aboriginal culture, historical contexts which may influence health, and relevant Aboriginal health and other national and state policies/strategies.
  4. Knowledge and understanding of local and regional Aboriginal and mainstream health and community services and the demonstrated ability to take initiative, work autonomously and develop, implement and evaluate referral systems utilising a multidisciplinary approach.
  5. Demonstrated ability to implement changes and develop direction in clinical practice by the initiation, utilisation and review of evidence based research and experience with contributing to strategic and clinical service planning.
  6. Demonstrated high level advanced communication and interpersonal skills (including the use of coaching) across a variety of mediums and proven ability to write for a variety of documents. This includes the ability to develop rapport and trust with Aboriginal people and advocate on behalf of the patient, carers and families whilst exercising good judgement and cultural appropriateness, with the ability to maintain confidentiality and respect
  7. Understanding of and commitment to the effective and meaningful engagement of clinicians in the decision making process that impacts on positive patient outcomes.
  8. Valid unrestricted drivers licence for use in NSW/Australia and willingness to travel in the course of employment.
