- Degree in Social Work with eligibility to join the AASW or Degree in Occupational Therapy or Physiotherapy with AHPRA registration, Registered Nurse – Division 1 Registered Nurse (APHRA registered) . Current Australian Drivers License.
- Demonstrated clinical knowledge and experience in Aged Care and the ability to apply this in acute, sub-acute, Residential Aged Care Facility and community settings.
- Demonstrated ability to work in a multidisciplinary environment and engage effectively with members of a multidisciplinary team.
- Demonstrated ability to work independently and in an effective manner encompassing appropriate initiative, organisational, problem solving and time management skills.
- Demonstrated experience in completing holistic functional assessments of older people with complex health needs.
- Demonstrated ability to identify appropriate community based services and supports (including Residential Care) for Aged Care Clients and facilitate access to these as required.
- Demonstrated knowledge of the key components of Department of Health and Ageing ACAP guidelines.
- Demonstrated understanding of how the District is Caring for the Coast and how this role would contribute.
