- Tertiary qualification or equivalent work experience in a health related field.
- Demonstrated understanding of the provision of health care for people with chronic disease conditions and/or complex health needs, with relevant experience such as working in chronic care services and/or community services.
- Demonstrated experience in coordinating/case managing the care of patients with chronic disease and/or complex health needs.
- Demonstrated competent high level communication and interpersonal skills, report-writing, with an ability to engage various members of the multidisciplinary team and a wide range of people from a variety of backgrounds.
- Demonstrated creativity and innovation in approach to complex problem solving with the ability to negotiate, network, multi-task and manage competing deadlines.
- Demonstrated proficiency in using computer systems in a Windows environment, including MS Office applications and common database/statistics software.
- Understanding of the Ministry of Health Integrated Care Strategy and how it relates to the JH&FMHN environment.
