Patient Journey Facilitator

Selection Criteria:

1. Current registration with the Australian Health Practitioners Regulation Agency (AHPRA).
2. Broad and extensive recent clinical care experience in an acute healthcare setting with proven coordination skills in a complex environment.
3. Excellent communication skills, both written and verbal with the ability to relate to senior medical and clinical staff.
4. Demonstrated leadership and capability in implementing a person-centred model of care in a multidisciplinary team environment.
5. Broad knowledge of clinical pathways, expected length of stay and resource utilisation.
6. Demonstrated ability to work independently and as a member of an interdisciplinary team.
7. Broad knowledge of the NSW Ministry of Health Whole of Hospital Program and ability to implement sustainability strategies related to the program.
8. Demonstrated computer skills including the ability to effectively use and generate reports from the patient administration system, patient flow portal, and electronic medical record.
