Join the Transitional Aged Care Program (TACP), a dedicated and dynamic group of healthcare professionals, including Enrolled Nurses, Registered Nurses, Physiotherapists, Occupational Therapists, and Domestic Assistants. Work within a supportive environment, offering community-based care to clients striving to regain independence in their own homes.
About the role:
As an Enrolled Nurse in the TACP, you will play a vital role in assisting clients to achieve their recovery goals. This short-term (up to 12 weeks) Commonwealth-funded program supports older individuals leaving the hospital who would otherwise require residential aged care but have the potential to remain at home with the right assistance.
Your responsibilities will include:
- Conducting assessments and delivering tailored care plans.
- Working closely with a multi-disciplinary team to optimize patient outcomes.
- Providing care within a 7-day-per-week service, including some weekend shifts.
- Engaging in face-to-face training and receiving ongoing support from Registered Nurses, Enrolled Nurses, the NUM, and the Service Manager.

- The ability to work independently and as part of a multidisciplinary team.
- A proactive approach to problem-solving and patient care.
Requirements:
Our successful candidate will possess the following:
- Current registration as an Enrolled Nurse (medication endorsed where applicable) with current Authority to Practice with AHPRA.
- Unrestricted driver’s licence and willingness to travel for work.
