We have an exciting opportunity for a Community Aged Care Aide to work in a dynamic team leading our approach to the provision of quality care to older people. You will work the multidisciplinary team to provide therapeutic intervention and support both in the community health centre and in their homes. The Community Aged Care Aide will assist clients in task such as transport to Aged Care Clinics, completing exercise programs, delivering, and installing equipment and aides to support independence. The role provides support and assistance to the multidisciplinary teams with the Aged Care Service working directly alongside team members to provide a comprehensive service to older people in the community.

What you'll be doing
Deliver clinical care and clinical support as delegated by the supervising Allied Health Professional or Registered Nurse to achieve client outcomes and document relevant intervention as appropriate in client files.
