About the Role

In this role, you will be expected to have demonstrated experience providing operational management of frontline clinical services working in community. You will be expected to have experience providing clinical leadership in child and family services, Youth Drug Health Services, or other community outreach services.

 

You will work in settings across Sydney Local Health District in line with Ministry of Health policy and guidelines and evidence-based practice. You will pursuethe optimal performance of the programs through effective leadership of multidisciplinary teams and the collaborative development of strategic alliances with relevant stakeholders including consumers, health services, other government departments/agencies and private and non-government organisations.

 

The core functions of the multidisciplinary community based team are:

  • Provide care coordination for young people with drug & alcohol issues, as informed by their professional discipline
  • Facilitate the development of care plans in consultation with all relevant parties utilising harm reduction principles and best practice models
  • Provide referral for young people with drug & alcohol issues to appropriate support services
  • Provide support for other staff (mainstream, drug & alcohol, Youth Sector etc.) in relation to youth drug and alcohol issues
  • Support education and health improvements initiatives for young people experiencing drug and alcohol issues and develop links and partnerships with other services

 

The team works in the community setting and aims to provide a safe platform for open dialogue and assists in the development of strategies to empower young people and their families and or carers in their journey towards recovery.

Ideal Candidate

  • Current nursing registration with the Nursing and Midwifery Board of Australia (AHPRA) and 5 years full time equivalent post registration experience.
  • Holds approved postgraduate qualifications relevant to the specialty or such other qualifications or experience deemed appropriate.
  • Leadership skills and excellent interpersonal, oral and written communication skills.
