Join us at the forefront of healthcare reform in NSW as we implement a refreshed approach to Collaborative Commissioning. As a Project Officer in the Connected Care team, you will play a key role in strengthening partnerships between hospitals, primary care, and community services to deliver more connected, patient-centred care. Working closely with health services, Primary Health Networks, consumers and internal Ministry teams, you will help design and deliver innovative, practical solutions that bridge gaps in care. The Project Officer supports the development and implementation of a range of projects and policy initiatives that contribute to achieving the Connected Care Team’s priorities in alignment with Government objectives. The role performs project management and policy support activities and provides informed advice to enable the System Performance Support Branch to oversee reforms that support the future vision for the health system. The System Performance Support Branch team works in close partnership with health service executives, senior managers, and clinicians, collaborating closely with other Ministry of Health and health pillar staff to support the performance and sustainability of health services.


A key focus for the Connected Care team is implementing new partnership approaches to support people at risk of hospital attendance and assist individuals reintegrating into the community after hospital care. Connected Care has the ability to inform, connect, personalise, accelerate, and complement existing services to deliver care across the continuum — from disease prevention and treatment to ongoing monitoring. Through the refreshed Collaborative Commissioning approach, there is a significant opportunity to relieve pressure on Emergency Departments and hospitals by delivering integrated care solutions that are locally-driven and patient-focused.


Health Manager level 2, starting salary from $109,857 - $129,624 plus Super, with a 40-hour work week.


What you’ll be doing

  • Support the Senior Manager and broader Connected Care team to implement the refreshed Collaborative Commissioning approach, including preparing reports, maintaining project documentation, and monitoring project milestones across Local Health District and Primary Health Network partnerships.
  • Assist with procurement and management of commissioned services and support contracts, ensuring deliverables align with the objectives of Collaborative Commissioning initiatives.
  • Provide timely advice to managers and executives on operational and strategic issues impacting the delivery of Collaborative Commissioning projects, drawing on stakeholder feedback and emerging best practice.
  • Conduct research and analysis on local models of care, service integration challenges, and partnership outcomes to develop evidence-based recommendations that drive program improvements.
  • Build and maintain strong working relationships with LHDs, PHNs, and other partners through effective communication, consultation, negotiation, and problem-solving to support the achievement of collaborative, system-wide care reform goals.


About you


We are seeking motivated and committed individuals who can support the work of NSW Health with:

  • Demonstrated experience gained in a health related industry, working effectively and cooperatively within a team environment with the ability to work autonomously.
  • Demonstrated knowledge and experience in health related project management with the ability to plan, organise, and implement projects on time.
  • Demonstrated excellent organisational skills, the capacity to successfully complete several tasks concurrently and to achieve competing deadlines.
  • Demonstrated high level and effective communication skills (both verbal and written) including the ability to synthesise and present complex concepts to diverse audiences.
  • Demonstrated ability to prepare complex reports and a wide range of government documents, which include reporting of data.
