At Rankin Park Centre, you’ll work in a close-knit Social Work team as part of a respected multidisciplinary health team. Where you'll be working:

Rankin Park Centre is a rehabilitation facility comprising a 40-bed inpatient unit and a Day Hospital outpatient service. You will be working in a small Social Work team which is part of a close working multidisciplinary health team where all members are valued and respected for their individual input. The Social Work team offers support and supervision to enhance skill development in psychosocial assessment, NDIS applications and planning, Aged Care, Guardianship and capacity assessment, report writing, grief and loss support for both patients and their family members. The opportunity is available to work within the context of Psychoeducational groupwork for several casemix groups.


What you'll be doing:

The position provides a mixed caseload, in a fast-paced environment with challenges that allow for growth and learning about different services, such as accommodation and housing, legal issues, financial support, domestic support, community access, family support etc. One staff member reports “you never stop learning in this field; no situation is the same and you learn something from each patient and every situation”. 

You will be liaising with a variety of community organisations and service providers and providing handovers and /or reports as needed to ensure seamless flow of care for every patient.

Every week you will come together with the multidisciplinary team at a case conference to discuss each patient’s situation and troubleshoot the best outcome in the best time frame to support a safe and timely discharge.

Initially you would be working in the inpatient environment with a focus on safe and supported discharge for patients, however there may be an opportunity to work within the Day Hospital as part of the team working with outpatients requiring rehabilitation inputs.

You will be patient centred, a good advocate, with great negotiating skills to achieve the best outcome for your patients and their families and key support people. You will be keen to learn and to participate in improvement activities that benefit our service and its clients. You will be a good communicator and enjoy working in a team environment, sharing your knowledge but open to learn from others. You will be organised, flexible and good at managing priorities that may shift throughout the day and week.
