Where you'll be working:
We’re excited to offer a brand-new Social Work position at John Hunter Hospital (JHH), working alongside a newly established Occupational Therapy role and the NDIS Lead Manager. In this dynamic role, you’ll collaborate with ward staff to support patients with complex needs, helping to streamline discharge planning and reduce hospital stays.

As part of the Social Work Department at JHH, you’ll receive professional support while reporting to the NDIS Lead Manager (currently the Service Manager for Occupational Therapy) for your day-to-day activities. This is a unique opportunity to make a real impact in a multidisciplinary team dedicated to improving patient outcomes.

About the role:
No two days will be the same in this dynamic role! Your daily activities will be shaped by the evolving needs of inpatients, hospital priorities, and the development of effective escalation plans. You'll play a key role in connecting patients with essential support services, guiding staff through complex problem-solving, and ensuring smoother transitions of care.

Additionally, data capture and reporting will be crucial in shaping and solidifying this position, helping to demonstrate its impact and embed it as a vital part of the hospital's support network.

A professional development program will be developed through consultation with the successful applicant. There is a lot of support available through the Medical and Surgical Teams Team Leaders and broader social work dept through to the Social Work Service Manager and the Service Manager NDIS Lead, LHD NDIS Program Coordinator.

About you:
You will have a good understanding of NDIS processes and services and challenges, especially from a Hospital / Health perspective. You will be able to communicate effectively with a variety of clinicians and service providers and enjoy the challenge of establishing a new position and have good report writing and data collection skills.

Requirements:
  • Social Work Bachelor Degree or qualifying Masters Degree providing eligibility for membership of the Australian Association of Social Workers (AASW).
  • Eligibility to drive in NSW.
