Where you’ll be working

Join the Transitional Aged Care Program (TACP), where you will be part of a dynamic multidisciplinary team dedicated to supporting older adults transitioning from hospital to home. This program provides evidence-based assessment and intervention to clients and their carers, helping them regain independence and improve their quality of life. You’ll work closely with experienced health professionals in a collaborative and supportive environment to deliver high-quality care.

About the role

As an Occupational Therapist within TACP, you will:

  • Provide advanced assessment and intervention for older adults in rehabilitation and aged care settings.
  • Support clients in their transition from hospital to home, ensuring they have the skills and supports needed for independent living.
  • Work within a multidisciplinary team, collaborating with nurses, physiotherapists, social workers, and other allied health professionals.
  • Manage a diverse caseload, requiring strong time management, flexibility, and adaptability.
  • Work both autonomously and as part of a team, making a meaningful impact on the lives of older adults and their carers.

About you

We are looking for an experienced Occupational Therapist who:

  • Has advanced clinical skills in aged care and rehabilitation.
  • Is flexible, adaptable, and highly organised in managing workload demands.
  • Works effectively in both team-based and independent settings.
  • Demonstrates excellent communication and interpersonal skills.
  • Is passionate about enhancing the independence and well-being of older adults.
