Moree is a vibrant town located in the beautiful Gwydir Valley in northwest NSW and situated on the banks of the Mehi River.  Moree is a place for relaxation and rejuvenation; well known for the hot artesian springs available to soak in, sourced naturally from deep inside the Great Artesian Basin and naturally heated to 41 degrees Celsius.  The rich soil surrounding Moree supports delicious paddock-to-plate produce, from olive groves and vineyards to pecan nut farms, cotton and beef.

About the role:

Moree Community Health is seeking a dedicated Diabetes Educator and Cardiac Rehabilitation Clinical Nurse Specialist Grade 2 (CNS2) to provide individualised, case-managed complex client care and leadership for specialty clinical practice and service delivery within a health service environment to maximise the health outcomes for the client. Specialty areas include Diabetes Management and Cardiac Rehabilitation. This role involves community and acute support for Diabetes Management and Cardiac Rehabilitation in Moree Health Services. Outreach for Diabetes Management support also occurs for clients at Warialda, Bingara, Mungindi, Boggabilla and Toomelah.

This position is also involved in the Medical Outreach Indigenous Chronic Disease (MOICD) Program, for Diabetes in collaboration with a Dietician and Aboriginal Health Worker. You will be supported with online learning resources and mentoring available.


About the candidate: 

You are a motivated and autonomous individual who is skilled in diabetes management and seeking an opportunity to gain skills in cardiac rehabilitation in a community-based position. You have a postgraduate qualification in the field of Diabetes Management and/or Chronic Care with 3 years postgraduate experience. Consideration will be given to applications with less experience within the field of Diabetes Education who have completed or will commit to completing relevant postgraduate qualifications within 12 months of appointment - these applicants will be graded and remunerated in line with qualifications upon commencing and will progress to CNS2 grading once requirements are met.
