About the role:
Responsibilities include:
- Providing expert advice to community clients and carers, enhancing dementia awareness post-diagnosis, and supporting strategies for home-based care.
- Consulting with community service providers, establishing respite care networks, and minimising hospital presentations.
- Conducting assessments and referrals to medical specialist and community services as required.
- Liaising with Specialist Geriatricians for optimal client outcomes.
- Monitoring medication compliance, providing feedback to medical staff, and coordinating with GPs to ensure continuity of care.
- Engaging in proactive follow-ups to prevent relapse and readmission.
- Collaborating with Dementia Australia, Dementia Support Australia, and other health services to provide resources and facilitate access to necessary assessments and treatments.
- Acting as an ambassador for health services to foster positive client and family experiences.
- Assisting clients to access health services, attend necessary appointments, investigations and testing for assessment.

At Taree Community Health Service, we are proud to provide a range of healthcare and support services to the local community, focusing on preventive care, chronic disease management, and well-being initiatives. Our Aged Care Service consists of a Psychogeriatric Service of a CNC and Registered Nurse. The team also boasts a Nurse Practitioner specialising in Aged Health & Diseases, along with access to a Specialist Geriatrician. You will be supported by the Aged Care Service Manager, administrative staff and a large experienced multi-disciplinary team. Regular online education is required for the safety of all staff, along with the support to attend further ongoing education and relevant professional development.
Requirements:
- Registered Nurse with current Authority to Practice with AHPRA with at least 5 years full time equivalent post registration experience and in addition who has approved post registration nursing qualifications in Postgraduate in Geriatric Nursing/ Mental Health Nursing
- Eligibility to drive in NSW with current unrestricted driver’s licence and the ability and willingness to travel for work purposes which may involve driving long distances

- To be eligible for permanent employment in this position you must be an Australian Citizen, or a permanent resident of Australia, or a New Zealand citizen with a current New Zealand Passport.
