About the role:
This role will see you provide expert clinical organisation, leadership, coordination, and consultancy in dementia care within the Lower Mid North Coast Sector of Hunter New England Local Health District.
Responsibilities include:

  • Providing expert advice to community clients and carers, enhancing dementia awareness post-diagnosis, and supporting strategies for home-based care.
  • Consulting with community service providers, establishing respite care networks, and minimising hospital presentations.
  • Conducting assessments and referrals to medical specialist and community services as required.
  • Liaising with Specialist Geriatricians for optimal client outcomes.
  • Monitoring medication compliance, providing feedback to medical staff, and coordinating with GPs to ensure continuity of care.
  • Engaging in proactive follow-ups to prevent relapse and readmission.
  • Collaborating with Dementia Australia, Dementia Support Australia, and other health services to provide resources and facilitate access to necessary assessments and treatments.
  • Acting as an ambassador for health services to foster positive client and family experiences.
  • Assisting clients to access health services, attend necessary appointments, investigations and testing for assessment.

At Taree Community Health Service, we are proud to provide a range of healthcare and support services to the local community, focusing on preventive care, chronic disease management, and well-being initiatives. Our Aged Care Service consists of a Psychogeriatric Service of a CNC and Registered Nurse. The team also boasts a Nurse Practitioner specialising in Aged Health & Diseases, along with access to a Specialist Geriatrician. You will be supported by the Aged Care Service Manager, administrative staff and a large experienced multi-disciplinary team.  Regular online education is required for the safety of all staff, along with the support to attend further ongoing education and relevant professional development.

Requirements:

  • Registered Nurse with current Authority to Practice with AHPRA with at least 5 years full time equivalent post registration experience and in addition who has approved post registration nursing qualifications in Postgraduate in Geriatric Nursing/ Mental Health Nursing
  • Eligibility to drive in NSW with current unrestricted driver’s licence and the ability and willingness to travel for work purposes which may involve driving long distances

  • To be eligible for permanent employment in this position you must be an Australian Citizen, or a permanent resident of Australia, or a New Zealand citizen with a current New Zealand Passport.
