About us:

The Sustaining NSW Families Program (SNF) is a coordinated and integrated program of sustained nurse led home visiting to improve outcomes for vulnerable families and children. The successful candidate will be joining a multi-disciplinary team of motivated and enthusiastic clinicians to continue this program across Newcastle, and the Lake Macquarie area.  The team culture is one built on respect, collaboration, compassion and balanced with a friendly, fun atmosphere.



About the role:

The Sustaining NSW Families Program model involves working in partnership with families to achieve goals and desired outcomes agreed in consultation with the nurse whilst keeping the baby in mind. This is achieved through ongoing and active follow-up with families by attending structured home visits. There is weekly consultations and care discussions with the multidisciplinary team through case conferencing to review care plans.

A family partnership and anticipatory guidance approach is utilised to promote optimal infant and maternal mental health, child development and positive parent child relationships.

The role is supported by the Manager for Child and Family Services, NUM/Coordinator for SNF, Allied Health team and nursing colleagues. Orientation to the program will be provided on commencement. All SNF nurses are required to maintain mandatory education requirements. All SNF nurses are required to complete the Advanced Nurse Practice (ANP) Course. This includes a case study assessment and attendance at training days. The SNF program employs a ANP Course Coordinator to provide support with ongoing training required.


About You:

The successful candidate will be enthusiastic with a passion for working with vulnerable families. The candidate will need to demonstrate strength as a reflective practitioner, work in a partnership approach as well as being able to form therapeutic relationships built on health boundaries. The preferred candidate will need to be prepared to attend the education and training for this position.


Requirements:

  • Current Authority to Practice as a Registered Nurse and/or Registered Midwife with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Holds postgraduate qualifications in Child and Family Health Nursing and willingness to complete an Advanced Nurse Practice course within 12 months of employment.
  • Eligibility to drive in NSW and willingness and ability to travel for work purposes.
