About the Role

We are seeking a motivated Aboriginal Chronic Care Project Officer to support and coordinate key chronic disease outreach programs within the HNE Aboriginal Chronic Care Team. This role will involve a mix of data entry, planning, audits, service rounding, and site visits with managers—with a strong focus on improving health outcomes for Aboriginal communities.

In this role, you’ll start with a gradual onboarding process, including buddying with team members for site visits, before stepping into independent tasks. We will invest time in mentoring and providing ongoing support to help you succeed.

Where You'll Be Working

You will be part of a strategic team within the Hunter New England Health Aboriginal Health Unit, Aboriginal Chronic Care team, working closely with program managers and Aboriginal health professionals dedicated to improving chronic care services for Aboriginal communities across the district.

We are looking for someone who:

  • Has strong analytical thinking skills and a keen eye for detail.
  • Is connected to community and passionate about improving Aboriginal health outcomes.
  • Can manage multiple priorities while meeting deadlines.
  • Holds a current driver’s licence and is willing to travel, including occasional overnight stays.

Some skills can be learned on the job, but community connection and a commitment to culturally safe care are essential.
