Your Impact
As the Comprehensive Palliative Care in Aged Care (CPCiAC) Project Officer, you’ll lead a district-wide scoping and needs analysis to improve access to specialist palliative and end-of-life care for older people in Residential Aged Care Homes (RACHs).

You’ll contribute to shaping a person-centred model of care by identifying service gaps and developing strategies to enhance  access to specialist care, workforce capacity, equity, and streamlined referral pathways. This includes project planning, stakeholder engagement, and milestone reporting.


Your Experience and Qualities
You bring:

  • Strong project management skills in complex service environments
  • Excellent communication and stakeholder engagement abilities
  • The ability to analyse service needs and drive improvement initiatives
  • High-level organisation and time management
  • Advanced computer skills to support delivery and reporting
    Knowledge of aged or palliative care is desirable, but a willingness to learn is just as important.

Additional Information:
This position is full time; however, job share and/or part time arrangements will be considered.
