About the Role 

Working Monday to Friday, you'll collaborate with a small team of skilled Child and Family Health Nurses to deliver safe, culturally responsive, and person-centred care. This role combines clinic-based services and home visits, including visits in the crucial first two weeks following birth.

You'll support families from birth to five years, assessing developmental milestones and offering tailored care and guidance. You’ll act as an advocate, empowering families and carers to make informed decisions and connecting them with appropriate services to support their health goals.

Key Responsibilities 

  • Provide developmental assessments for children aged 0–5 years.
  • Conduct home visits and clinic-based consultations.
  • Work collaboratively within a multidisciplinary team.
  • Offer health education and support to families and carers.
  • Make timely and appropriate referrals to support client outcomes.
