This position qualifies for an annual Recruitment Incentive Package up to the value of $10,000 (conditions apply and may be pro-rata’d to your contracted or ordinary working hours, these payments are paid on a fortnightly basis) under the NSW Health Rural Health Workforce Incentive Scheme.

This scheme recognises the role key health workers play in enabling the delivery of rural and remote health services, and aims to provide incentives over and above award entitlements, to attract and retain staff in these areas.

What you'll be doing
 

  • Provide a high quality Social Work service to patients, clients, consumers and their family/carers
  • Ensure the provision of optimal health outcomes consistent with FWLHD policies, procedures, standards, relevant legislation, and  the NSW Health Code of Conduct. 
  • Enhanced Care Team as part of the Integrated Care Team.

How to apply

To be considered for this position, please ensure you address the selection criteria in the application as thoroughly as possible:

  • A bachelor degree in Social Work which provides eligibility for membership of the Australian Association of Social Workers and a minimum of 3 years post graduate experience in a relevant field. Knowledge, skills and experience to meet the requirements of the NSW Health Professionals Award Level 2.
  • Demonstrated ability to work in a health related field and ability to provide a range of Social Work interventions eg psychosocial assessment, case management ,group work, advocacy and education
  • Demonstrated ability to work with Bereavement, Grief and Loss in a Palliative Care setting and an understanding of current grief and loss theory.
  • Demonstrated ability to work with consumers, families and carers in a time of high levels of family stress.
  • Extensive specialist or broad generalist knowledge, clinical proficiency and ability to work with minimal supervision, and collaboratively as a member of a multidisciplinary team.
  • Ability to effectively prioritise and organise own work/ caseload.
  • Demonstrated commitment to professional development, quality improvement and evidence based practice.
