The CCLHD Social Work Department is seeking an experienced Level 1-2 Social Worker to join the Inpatient Social Work team at Wyong Hospital.

This position currently provides a Social Work service to the Medical Assessment Unit, Medical Assessment Unit, 6 days a week, on all days of the year except Christmas day.

This is a permanent part time position of 24 hours per week and covers Thursday, Friday and alternate Wednesday 0800 to 1630 and on 4 hours on Saturdays.

Primarily the position in the MAU provides rapid and early assessment and counselling to assist patients and their families and/or carers, and undertakes case planning, liaison, advocacy and referral to ensure access to appropriate community services. The position works closely with the Critical Care team and provides cover to the Emergency Department and the Intensive Care unit as required, as well as other clinical priority identified areas in the hospital

Participation in a Social Work On Call roster (at either Gosford or Wyong Hospital) is a requirement of this position. For new staff, this would not be until after approximately 6 months in the position and following a training program. This participation is usually 2 shifts per month.  Please read the On Call information attached to this position description.

What you'll be doing
Provide a professional social work service to patients and their family/carers to ensure the provision of optimal health outcomes for patients. The position is currently based at Wyong Hospital in the Medical Assessment Unit. This position will work within a dedicated interdisciplinary team and with the other inpatient social workers to deliver high quality care in a dynamic team environment.

Primarily the position is to provide assessment and counselling to assist patients and their carers to deal with the emotional impact of illness and to undertake case planning, liaison and advocacy and referral to ensure access to appropriate community services. The social worker is to work closely with interdisciplinary colleagues and senior managers within the hospital to ensure the development and delivery of effective and pro-active social work services. The social worker will be expected to be active in the areas of service development, service evaluation and staff and student teaching.
