Are you an experienced and compassionate Social Worker ready to make a difference in the lives of patients and their families during times of illness, crisis and/or trauma and keen to mentorship and support your Allied Health and Multi-Disciplinary Team (MDT) colleagues in enabling timely and appropriate discharge for patients who are experiencing complex needs? Join our dynamic team at Central Coast Local Health District (CCLHD) as Senior (Level 3) Social Worker as part of the CCLHD Complex Discharge Advisory Team (CDAT) and be a crucial part of delivering optimal health outcomes and psychosocial care.
As a Social Worker within the CDAT program you will have a unique opportunity to lead and implement service innovation. This service integrated program brings both Social Work and Occupational Therapy together to provide consultation, to mentor, build practice capacity and work alongside Allied Health (AH) and multi-disciplinary team (MDT) colleagues and patients and their families/carers to enable timely and appropriate discharge with sustainable transfer of care to the community.
With advanced understanding of:
- complex psychosocial challenges that can occur for patients and/or their families and the impact these challenges have upon timely and appropriate hospital discharge;
- the range of emotional responses that can accompany chronic illness, crisis and trauma for patients and their families;
- approaches that enables mentoring and support provided to your allied health and multi-disciplinary team (MDT) colleagues;
- the community service system including Guardianship and the National Disability Insurance Scheme (NDIS) processes;
You'll develop internal and external service partnerships, provide comprehensive assessments, care and discharge planning, mentorship, support and guidance to your MDT colleagues. Your counselling and advanced Social Work practice skills and your ability to adapt communication styles work collaboratively in multidisciplinary teams, and prioritize competing demands will ensure the delivery of high-quality care.
About You
You have tertiary qualification in Social Work, with eligibility for membership of the Australian Association of Social Workers (AASW) (overseas qualifications must supply the AASW Overseas Assessment number). A demonstrated understanding of the current AASW Practice Standards as they relate to Inpatient Social Work and a commitment to maintaining a professional skill and knowledge base.
You possess a well-developed understanding of the psychosocial issues that arise for patients and their families during chronic and acute illness and trauma. Significant demonstrated social work experience in the clinical specialty area where applicable.
You have demonstrated experience in providing psychosocial assessment and crisis management, within person centred, trauma informed and brief intervention models and preferably within a health clinical setting and or within the clinical specialty where applicable.
You have Demonstrated experience and skill in providing leadership, supervision and mentor support to staff and students as well as experience as a proactive senior member of a multidisciplinary team.
Where you'll be working
The position is part of the Inpatient Social Work team based at Wyong Hospital that provides a professional social work service to patients and their family/carers to ensure the provision of optimal health outcomes for patients consistent with CCLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct. This position will work alongside the multidisciplinary teams to deliver high quality care in a dynamic team environment.
