The Central Coast Local Health District ( CCLHD) is looking for a dedicated Registered Nurse to provide person centred care within the Osteoporosis Re-fracture Prevention & Menopause Service & work collaboratively with the multidisciplinary team to achieve identified goals & optimal health outcomes.
About You
- You have a current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
- You have relevant clinical experience in Women’s health with demonstrated knowledge and understanding of the menopause process or knowledge and experience in bone health, with a commitment to quality improvement, practice development and evidence-based practice.
- You can demonstrated effective clinical communication using information technology including use of Virtual Care, and sound written, verbal and interpersonal skills with ability to adapt communication style to the audience.
- You hold a Drivers Licence
You will be required to:
- Conduct bone health assessments, education, personalised management plan & refer consumers to appropriate services as per Osteoporosis Re-fracture Prevention Service (ORP) referral pathways.
- Support women in accessing appropriate bone health care including accessing internal & external services, provide resources & support to make healthy behaviour changes.
- Conduct menopause nursing assessments, education & development of a personalised management plan in consultation with the multi-disciplinary team.
- Support women in accessing appropriate menopause care including accessing internal & external services as per menopause referral pathways, provide resources & support to make healthy behaviour changes.
- Provide follow up assessments over time to ensure individual goals are met & adherence to treatment plans.
RESPIRATOR USE
NSW Health workers may be required to use a respirator, as part of their appointment with NSW Health. Where a respirator is required for use, workers will be instructed in their safe use, including donning, doffing and fit checking. Staff may be required to complete fit testing to selected respirator/s to assess their facial fit/seal. At all times when a health worker is required to use a respirator, the health worker must not have any facial hair present. Processes are in place to support workers that need to keep facial hair due to religious observance requirements and/ or health conditions.
