In this rewarding role, you will deliver a broad range of clinical services including wound care, palliative care, catheter and continence management, CVAD care, and drain management.

About the Role:
- Ongoing health monitoring of individuals and communities.
- Health promotion and maintenance, early disease detection, and timely intervention.
- Rehabilitation and palliative care, supporting optimal function or comfort.
- Care planning and case management that empowers clients and supports families.
- Developing responsive strategies to adapt to the changing health needs of the community.
- You’ll be part of a multidisciplinary team committed to promoting health equity and improving outcomes through client-centred, culturally safe, and socially responsive care.

About You:
You are a motivated and compassionate nurse who:
- Holds current registration with AHPRA as a Registered Nurse.
- Hold a current NSW drivers Licence and able to travel around the Central Coast
- Demonstrates a strong understanding of primary health care principles and community-based care.
- Can assess, plan,deliver and evaluate care across various community health scenarios.
- You hold recent nursing experience in Community Nursing or a demonstrated interest in Community Nursing.
