Are you seeking a career that allows you to make a profound difference in the lives of others? Join our End-of-Life Community Team at Central Coast Local Health District (CCLHD), where you’ll play an integral role in providing compassionate, patient-centered care to individuals and families during one of the most important stages of life.

Why This Role is So Rewarding

Being part of the Community Specialist Palliative Care team means having the opportunity to support patients and their families through one of the most vulnerable and emotionally complex times. In this role, you will be making an impact by delivering care with dignity, empathy, and professionalism. The work you do will bring comfort, solace, and critical support to those facing the end-of-life journey, creating a lasting and meaningful difference.

This is a maternity relief position and should the permanent incumbent require an early return from maternity leave, this contract of employment will cease with two weeks notice. Maternity relief positions can be for a period of up to 12 months, with the possibility of continued employment for up to a further 12 months for maternity leave reduced hours, therefore there are no claims to permanency.

As a valued member of the team, you will have the opportunity to access other professional roles within the team and the broader organisation, as relevant to experience and qualifications.

What We’re Looking For:

  • Empathy & Compassion: A genuine desire to support patients and families through the challenges of end-of-life care.

  • Team Player: Strong collaborative skills, with the ability to work effectively within a multidisciplinary team.

  • Commitment to Excellence: Passion for providing high-quality, patient-centered care with a focus on dignity and respect.

  • Experience or Interest in End-of-Life Care: Previous experience in palliative care, grief support, or end-of-life services is desirable, but we are also eager to support professionals interested in this area of practice.
