What you will be doing
 

The primary purpose of the Transition Care Network Manager is to facilitate the development, coordination and evaluation of transition support programs for young people with chronic illnesses as they transition from paediatric to adult health care.  The manager will do this by leading the state-wide ACI Transition Care Service and providing high level strategic direction and advice on policies, programs and projects and high level advisory support as appropriate.

Remuneration: $127,384 - $152,407 per annum plus super

Selection Criteria:

 

  1. Demonstrated experience managing and influencing teams of multidisciplinary staff.
  2. Demonstrated experience in redesign methodology, project management and working with consumers and carers.
  3. Highly developed interpersonal and communication skills and demonstrated high level written and presentation skills including experience in policy development and research.
  4. Sound knowledge of the NSW Health System.
